Mounjaro at Soboba

Mounjaro (tirzepatide) represents the next generation of GLP-1-based weight loss medicine. Unlike semaglutide — which targets the GLP-1 receptor alone — tirzepatide is a dual-action agonist that simultaneously activates both the GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide) receptors. This dual mechanism produces a stronger, more sustained appetite-suppressing and metabolic effect than single-receptor medications, making it the most clinically powerful weight loss medication currently available.\n\nIn the SURMOUNT-1 clinical trial published in the New England Journal of Medicine, participants using tirzepatide 15 mg lost an average of 20.9% of their body weight over 72 weeks — results that had previously been associated only with bariatric surgery. At the highest dose, 63% of participants lost 20% or more of their body weight. For many patients, this translates to 40, 50, or more pounds lost under physician supervision.\n\nAt Soboba Medical Weight Loss in Orange County, Mounjaro is available as part of a fully physician-supervised weight loss program. Our experienced physicians evaluate every candidate individually, review your complete medical history, and build a tirzepatide titration schedule designed to maximize your results while minimizing side effects. Physician visits are included in your program at no additional charge. If you've tried other GLP-1 programs without reaching your goals, Mounjaro's dual mechanism may provide the step forward you need.

How It Works

1

Your Soboba physician evaluates your candidacy through a thorough initial consultation — reviewing medical history, thyroid function, cardiovascular profile, prior weight loss experience, and current medications — before starting tirzepatide.

2

Tirzepatide activates both the GLP-1 and GIP receptors simultaneously — producing a more potent appetite suppression effect than semaglutide alone, while also enhancing insulin sensitivity and glucose metabolism through the additional GIP pathway.

3

The program begins at a low starting dose (2.5 mg weekly) and titrates upward over 20 weeks to the maintenance dose, with each increase timed carefully to your tolerance to minimize nausea and GI side effects during the adjustment period.

4

In SURMOUNT-1, 63% of participants on the highest dose of tirzepatide lost 20% or more of their body weight over 72 weeks — a clinical outcome magnitude historically associated with bariatric surgery, not medication.

5

Monthly physician check-ins allow your Soboba physician to assess your response to the current dose, adjust as needed, provide nutritional guidance, and monitor for any clinically relevant side effects throughout your program.

6

Because Mounjaro operates through a dual mechanism, many patients who experienced a plateau or suboptimal results on semaglutide-based programs (Ozempic, Wegovy) find that tirzepatide provides meaningful additional progress toward their weight loss goals.

Who Should Consider Mounjaro

Tirzepatide is appropriate for patients who meet specific medical criteria. It is particularly valuable for those who have experienced suboptimal results on semaglutide — Mounjaro's additional GIP pathway frequently provides meaningful additional progress in patients who have plateaued. Only a physician consultation determines actual candidacy.

BMI ≥ 30, or BMI ≥ 27 with at least one weight-related condition (hypertension, type 2 diabetes, dyslipidemia, or sleep apnea)

Prior diet and exercise attempts have not produced adequate or lasting weight loss

Patients who have plateaued or had partial results on a semaglutide-based program (Ozempic or Wegovy)

Not appropriate for patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia type 2 (MEN 2)

Not appropriate for patients with a history of acute pancreatitis or current severe gastrointestinal disease

Not appropriate during pregnancy or for those planning to become pregnant during treatment

Side Effects and Tolerability

Mounjaro's side effect profile closely resembles that of semaglutide — most are gastrointestinal and occur during the dose titration period. The 20-week gradual titration schedule Soboba uses is designed to minimize these effects by allowing your body to adjust to each dose level before advancing further.

Common Side Effects

Nausea (most common, especially early in titration)
Constipation or diarrhea
Vomiting
Stomach discomfort or bloating
Decreased appetite
Fatigue during dose adjustment

Serious Risks (Rare)

!Pancreatitis — seek immediate care if severe abdominal pain develops
!Gallbladder disease
!Thyroid C-cell tumors — FDA black box warning based on animal data; human relevance not established. Contraindicated with a personal or family history of MTC or MEN 2.
!Hypoglycemia when used concurrently with insulin or sulfonylureas

Black box warning: Tirzepatide carries the same warning as other GLP-1 medications regarding thyroid C-cell tumors based on animal studies. It is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or MEN 2. Your Soboba physician will screen for this at your initial consultation.

Mounjaro Cost and Your Options

Brand-name Mounjaro (tirzepatide) is FDA-approved for type 2 diabetes management and can cost $1,000–$1,200 per month without insurance. Zepbound — the same active ingredient (tirzepatide) at the same doses — is FDA-approved specifically for chronic weight management. Your Soboba physician will determine which brand and dosing approach is appropriate for your health profile.

Compounded tirzepatide was previously an affordability alternative to brand-name Mounjaro and Zepbound. FDA policy changes in 2025 removed tirzepatide from the shortage-based compounding exemptions that had previously allowed broader access — availability is no longer routine. Please speak with your Soboba physician for the most current information on available medications and pricing.
